Office Visit Appointments:

To schedule an office visit appointment, please choose below the location where you would like to be seen and then select your provider of choice and date/time. Of note, all of our providers see patients in our Ypsilanti/Ann Arbor office and only select providers see patients at our three satellite offices (Canton, Chelsea, Howell). Procedure Appointments:

Appointments for procedures are scheduled through our Patient Portal. If you do not have a patient portal account, please click “Contact Us” and send a short message that reads “requesting invitation to create an account.” You will then receive an email allowing you to create a patient portal account. You can also call us at 734-434-6262 to schedule a procedure appointment.
